Abstract:
RESUMEN: ANTECEDENTES:
El infarto agudo al miocardio (IAM) es una de las principales causas de muerte en el mundo. En la actualidad, los tratamientos de reperfusión han demostrado los mejores resultados para el tratamiento del IAM dentro de su ventana terapéutica (12 horas), sin embargo, en nuestro país, pocas personas tienen acceso a ellos. Las terapias angiogénicas son posibles tratamientos alternativos, ya que han demostrado efectos benéficos en patologías de origen isquémico. La nitroglicerina (NTG) y la terapia de oxigenación hiperbárica (HBO) son tratamientos que han demostrado activar mecanismos inductores de angiogénesis.
MATERIAL Y MÉTODOS:
Para esta investigación, se utilizó un modelo experimental de IAM en ratas, en el cual se liga quirúrgicamente la arteria coronaria descendente anterior. Las ratas fuerón agrupadas en 6 grupos: A(Control), B (Controles IAM), C “Sham” (Cirugía sin IAM y sin terapia), D (IAM con HBO), E (IAM con NTG) y F (IAM con NTG y HBO). Los tratamientos fueron administrados tras la finalización del procedimiento quirúrgico, el grupo D, recibió 3 sesiones de HBO, al grupo E se le aplico un parche de NTG de liberación transdérmica, el cual fue cambiado cada 24 horas por 3 días y el grupo F, recibió ambos tratamientos. Al tercer día posterior al IAM, las ratas fueron sacrificadas, los corazones fueron extraídos para analizar el % de área miocárdica infartada (AMI) y llevar acabo el análisis inmunohistoquímico contra CD34, VEGFR-2, y ERK1/2.
RESULTADOS:
La HBO, la NTG, y la terapia doble (HBO+NTG) demostrarón disminuir el % del AMI contra el grupo control IAM (12.20±1.269,11.83±0.229,10.98±0.988, respectivamente vs 23.43±1.398), de igual forma demostrarón un incremento significativo de los marcadores angiogénicos CD34 y VEGFR-2 en comparación con el grupo control IAM. (CD34: 17.667±1.76,36.833±3.016,41.667±2.55 vs 8.667±0.494, VEGFR-2: 16.167±1.62,24.667±133,31.50±0.76 vs 5.333±0.62). En el caso de ERK 1/2, solo los grupos E y F mostraron un incremento significativo contra el grupo B (32.50±1.232,49.833±3.96 vs 19.833±1.078).
CONCLUSIONES:
Las tres terapias probadas (HBO, NTG y HBO+NTG) disminuyeron el % de AMI e incrementaron la angiogénesis, resultados prometedores, ya que disminuirían la disfunción miocárdica y mejorarían la remodelación cardiaca. Es necesario complementar nuestros resultados con pruebas funcionales cardiacas y evaluar los cambios angiogénicos estudiados en un corazón infartado con más días de evolución para incrementar la objetividad del estudio.
ABSTRACT: BACKGROUND:
Acute myocardial infarction (AMI) is one of the leading causes of death in the world. Currently, reperfusion treatments have shown the best results for the treatment of AMI within its therapeutic window (12 hours), however, in our country, few people have access to them. Angiogenic therapies are possible alternative treatments, as well as the beneficial effects in pathologies of ischemic origin. Nitroglycerin (NTG) and hyperbaric oxygenation therapy (HBO) are treatments that have been able to induce the mechanisms leading to angiogenesis.
MATERIAL AND METHODS:
For this research, it was used an experimental model of AMI in rats, in which the anterior descending coronary artery is surgically ligated. The rats were grouped into 6 groups: A (Control), B (IAM Controls), C "Sham" (Surgery without AMI and without therapy), D (AMI with HBO), E (AMI with NTG) and F (AMI with NTG and HBO). The treatments were administered after the surgical procedure, group D, received 3 sessions of HBO, group E was applied a NTG patch of transdermal release, which was changed every 24 hours for 3 days and Group F received both treatments. On the third day after the AMI, the rats were sacrificed, the hearts were extracted to analyze the % infarcted myocardial area (IMA) and the immunohistochemical analysis was carried out against CD34, VEGFR-2 and ERK1/2.
RESULTS:
The HBO, the NTG, and the double therapy (HBO + NTG) demonstrated to decrease the % of the IMA against the AMI control group (12.20 ± 1.269,11.83 ± 0.229,10.98 ± 0.988, respectively vs 23.43 ± 1.398), in the same way demonstrate a significant increase in the angiogenic markers CD34 and VEGFR-2 compared to the AMI control group. (CD34: 17.667 ± 1.76.36.833 ± 3.016.41.667 ± 2.55 vs 8.667 ± 0.494, VEGFR-2: 16.167 ± 1.62.24.667 ± 133.31.50 ± 0.76 vs 5.333 ± 0.62). In the case of ERK 1/2, only groups E and F showed a significant increase against group B (32.50 ± 1.232.49.833 ± 3.96 vs 19.833 ± 1.078).
CONCLUSIONS:
The three proven therapies (HBO, NTG and HBO + NTG) decreased the % IMA and increased angiogenesis, promising results, since they would decrease myocardial dysfunction and improve cardiac remodeling. It is necessary to complement our results with cardiac functional tests and to evaluate the angiogenic changes studied in an infarcted heart with more days of evolution to increase the objectivity of the study.
