ABSTRACT: Several reports demonstrated evidence of West Nile Virus (WNV) infection in birds, crocodiles,
mosquitoes and a human case in México. The fact that there are few human WNV cases reported
in Mexico is difficult to explain because of the large number of human cases reported in the
United States and the available evidence that WNV is widely distributed in animals in Mexico.
The aim of this study was to evaluate using serologic and molecular approaches the presence of
WNV in northeast Mexico in different groups of patients and healthy subjects.
Serum and cerebrospinal liquid samples were analyzed to identify Anti-WNV IgM Abs, Anti-
WNV IgG Abs, Anti-DENV IgM Abs and Anti-DENV IgG Abs by ELISA. We also performed
real time RT-PCR to detect WNV genome and the detection of neutralizing antibodies by PRNT.
The study cohort consisted of 1432 individuals (588 febrile patients, 44 encephalitic patients and
800 asymptomatic blood donors). All subjects were negative for WNV IgM and real time PCR.
Sixty subjects were reactive for dengue virus (DENV) IgM (16 blood donors and 44 febrile
patients). A subset (n = 425) of individuals was also screened by ELISA for flavivirus IgG. The
prevalence of flavivirus IgG in febrile patients, encephalitic patients and blood donors ranged
from 33-51%. A subset (n = 147) of sera reactive for flavivirus IgG was further tested by plaque
reduction neutralization test. Six individuals with no history of travel during the preceding 12
months were seropositive for WNV. Another 65 individuals were seropositive for DENV1 and 24
were seropositive for DENV2. This work provides important evidence to clarify some of the
hypotheses to explain the behavior of WNV in our country. The high incidence of DENV
antibodies in the studied groups allowed us to suggest that WNV infection in northeast Mexico is
less severe than in other populations because DENV antibodies limit WNV infection in this
region.
RESUMEN: Existe evidencia de la presencia del Virus del oeste del Nilo (VON) en aves, cocodrilos, mosquitos y humanos en México. Los escasos reportes de humanos infectados por el VON en México son difíciles de explicar debido a que en EUA se han reportado una gran cantidad de casos en humanos y a que existe evidencia de que en México el VON se encuentra ampliamente distribuido en animales. El objetivo de este estudio fue el de evaluar la presencia del VON en humanos del Noreste de México empleando métodos serológicos y moleculares. Se analizaron muestras de suero y líquido cefalorraquídeo para identificar anticuerpos de los tipos IgG e IgManti-VON y anti-Virus del Dengue (VD) por medio de ELISA. Se realizaron pruebas de PNRP para la detección de anticuerpos neutralizantes. También se realizaron pruebas de RT-PCR anidado y PCR en tiempo real para identificar el genoma del VON. Se incluyeron un total de 1,432 sujetos (588 pacientes con enfermedad febril, 44 pacientes con encefalitis y 800 donadores de sangre). Todos los sujetos analizados fueron negativos para la detección del genoma viral por medio de las pruebas moleculares. No se detectó ningún sujeto positivo para anticuerpos del tipo IgM anti-VON. Se analizaron 425 muestras para la detección de anticuerpos del tipo IgG anti-VON y la prevalencia de anticuerpos IgG-VON en pacientes febriles, pacientes con encefalitis y
donadores de sangre fue de 51%, 33% y 50% respectivamente. Se analizaron 147 muestras positivas para anticuerpos IgG anti-VON por medio de la prueba de PNRP y se identificaron 6 sujetos positivos para anticuerpos neutralizantes anti- VON, los cuales no tenían historia de haber viajado en los últimos 12 meses. Se identificaron 65 sujetos con anticuerpos neutralizantes contar el VD y 24 fueron positivos para el VD serotipo 2. Este trabajo pemitió esclarecer algunas de la
hipótesis para explicar el comportamiento del VON en nuestro país. La alta incidencia de anticuerpos contra el VD en la población estudiada nos permite sugerir que en el noreste del país la infección por el VON es menos severa que en otras poblaciones debido a que los anticuerpos del VD limitan la infección con el VON en esta región.
