RESUMEN: Antecedentes: El craving es el deseo intenso de satisfacer una conducta adictiva que condiciona el uso excesivo de la droga, la recaída y pérdida de control en su consumo. En los últimos años el uso de cocaína se ha colocado en los primeros lugares de preferencia entre la población. Existen varios estudios que han comprobado la efectividad de la Auriculoterapia con agujas en el tratamiento de la adicción a la cocaína, así como otros estudios en los que la efectividad ha sido inconsistente.
Objetivo: Evaluar la eficacia de la Auriculoterapia con tachuelas y agujas como coadyuvante en el craving de pacientes adictos a la cocaína.
Hipótesis: La Auriculoterapia es un método terapéutico efectivo como coadyuvante, que disminuye el craving en pacientes adictos a la cocaína.
Metodología: Se formaron 3 grupos con 18 pacientes adictos a la cocaína en internamiento en el Centro Municipal de Atención a las Adicciones de Netzahualcóyotl cada uno. Los tres grupos tuvieron un tratamiento de base que consistió en terapia cognitivo-conductual y Bromazepam (tabletas 3 mg), una cada 12 horas, durante 5 semanas. Se agregó al grupo 1 Auriculoterapia con agujas durante 20 minutos en los puntos Shenmen, Simpático, Hígado, Riñón y Pulmón superior, 2 veces a la semana. Se aplicó Auriculoterapia con tachuelas a permanencia en los mismos puntos al grupo 2, se revisaron 2 veces a la semana. Al grupo 3 se le proporcionó solamente el tratamiento convencional. Se les aplicó el test de craving de cocaína a los tres grupos en la primera, tercera y quinta semana del tratamiento.
Resultados: El análisis de los datos obtenidos del test de craving de cocaína demostraron que los pacientes del grupo experimental 2 tuvieron mejores resultados en cuanto al valor de la media (∆1-3 semana de tratamiento= 38.78, ∆3-5=16.28, ∆1-5=55.06; P=0.0001), seguido del grupo 1 (∆1-3 semana de tratamiento=23.22, ∆3-5=18.17, ∆1-5=41.39; P=0.0001), en comparación con el grupo 3 con tratamiento convencional solamente (∆1-3 semana de tratamiento=16.84, ∆3-5=17.16, ∆1-5=34; P=0.076).
Conclusión: Este es el primer trabajo que se realiza usando la Auriculoterapia con tachuelas como coadyuvante al tratamiento para controlar la adicción a la cocaína, y se concluye que este tratamiento es altamente efectivo en la reducción del craving, siendo esta estadísticamente significativa en comparación con la Auriculoterapia con agujas (también significativa) o el tratamiento convencional (farmacológico y psicológico). Por esto la Auriculoterapia con tachuelas podría ser una alternativa importante a las opciones actuales para el control de las adicciones.
ABSTRACT: Background: The craving is the intense desire to meet an addictive behaviour which conditions excessive use of drugs, relapse and loss of control in their consumption. In recent years the use of cocaine has been placed on top of preference among the population. There are several studies that have proven the effectiveness of the Auriculotherapy with needles in the treatment of cocaine addiction, as well as other studies in which the effectiveness has been inconsistent.
Objective: Assess the efficacy of Auriculotherapy with tacks and needles as coadjuvant in patients addicted to cocaine craving.
Hypothesis: The Auriculotherapy is an effective therapeutic method as coadjuvant, which lowers the craving addicted to cocaine patients.
Methodology: Is formed 3 groups with 18 addicted to cocaine patients in internment in the Municipal Center of attention to the addiction of Nezahualcóyotl each. The three groups had a basis which consisted therapy cognitive behavioural and Bromazepam (3 mg tablets), an every 12 hours, for 5 weeks of treatment. Added Group 1 Auriculotherapy needle for 20 minutes in points Shenmen, sympathetic, liver, kidney and lung superior, 2 times a week. Applied Auriculotherapy with tacks to stay in the same points to group 2, reviewed twice a week. Group 3 is only gave conventional treatment. Applied them the test of the three groups cocaine craving in first, third and fifth week of treatment.
Results: Analysis of data obtained from the cocaine craving test demonstrated that patients in the experimental group 2 had better results on the average value (∆1-3 week treatment = 38.78, ∆3-5 = 16. 28, ∆1-5 = 55. 06; P = 0. 0001), followed by
Group 1 (∆1-3 week treatment = 23. 22, ∆3-5 = 18. 17, ∆1-5 = 41 39; P = 0. 0001), compared to the 3 group with conventional treatment only (∆1-3 week treatment = 16. 84, ∆3-5 = 17. 16, ∆1-5 = 34; P = 0. 076).
Conclusion: This is the first work being done using the Auriculotherapy with tacks as adjuvant treatment to control the cocaine addiction, and it is concluded that this treatment is highly effective in reducing the craving, being this statistically significant compared to the Auriculotherapy (also significant) needles or conventional treatment (pharmacological and psychological). Why the Auriculotherapy with tacks could be an important alternative to the current options for the control of addictions.
