ABSTRACT: Cystic fibrosis (CF) is the most common recessive autosomic disease in the Caucasian population, with disease frequency of 1/3000 live borns, and carrier frequency of 1/25. The disease is characterized by an imbalance of electrolytes and water transport, affecting respiratory, intestine, biliary and pancreatic conducts. The affected gene is the Cystic Fibrosis Transmembranal Regulator (CFTR), of which more than 1500 mutations have been described. Although some mutations have been classified as high risk mutations, association between the genotype and the pulmonary phenotype is not well understood. In order to have a better understanding of this correlation, it have been studied genes so called modifier genes, that are those genes different from the primary disease gene, that have the capacity to influence the clinical phenotype of the disease. Variants in the genes that codify for the innate immune response protein MBL, the potent chemotactic IL-8, the proinflammatory citokine TNF? and the antiprotease AAT, have been widely studied and were proposed for this work as modifier genes of the CF pulmonary phenotype.
RESUMEN: La fibrosis quística (FQ) es la enfermedad autosómica recesiva más común en la población caucásica, con una frecuencia de 1/3000 recién nacidos vivos, y una frecuencia de portadores de 1/25. La enfermedad se caracteriza por un desequilibrio en el transporte de electrolitos y agua afectando vías aéreas distales, intestino, vías biliares y pancreáticas. El gen afectado es el Regulador de la Conductancia Transmembranal de la Fibrosis Quística (RCTFQ), del cual se han descrito hasta la fecha más de 1500 mutaciones. Aunque se han clasificado algunas mutaciones como de alto riesgo, la asociación entre el genotipo y el fenotipo pulmonar no está bien entendida. Para comprender mejor la correlación, se han estudiado a los llamados genes modificadores, que son aquellos genes diferentes al gen primario causante de una enfermedad, que tienen la capacidad de influir en el fenotipo de la misma. Variantes en los genes que codifican para la proteína de respuesta inmune innata MBL, el agente quimiotáctico IL-8, la citocina proinflamatoria TNF? y la antiproteasa AAT, han sido ampliamente estudiadas y fueron propuestas para este trabajo de tesis como genes modificadores del fenotipo pulmonar de la FQ.
