RESUMEN: Introducción: la aplicación sistemática de un score diagnostico de apendicitis miniminiza el error diagnostico. La apendicitis continúa siendo la causa más frecuente de dolor abdominal. La cirugía de urgencia médica más frecuente realizada es la apendicetomía, afectando al 6% de la población general y de estos el 2-3% corresponde a la edad pediátrica con una incidencia de perforación en esta misma población que varia entre el 30 y 60%.
Objetivo: Se tuvo por objetivo central evaluar la utilidad de la escala de Alvarado o índice de Mantrels en el diagnostico temprano de apendicitis para reducir las tasa de diagnósticos tardíos y/o falsos así como la relación de dicha puntuación con su reporte histopatológico que representa el estándar de oro de esta enfermedad.
Material y metodos: Se realizó un estudio prospectivo observacional descriptivo longitudinal en el servicio de urgencias pediatría del HGZ/UMF No. 8 “Dr. Gilberto Flores Izquierdo” en el periodo comprendido mayo 2009 a noviembre del 2009. El estudio de casos se analizó por estadística descriptiva, con medidas de tendencia central y de dispersión para variables cuantitativas y porcentajes para variables cualitativas, valor predictivo positivo, valor predictivo negativo, curva de roc.
Conclusión: los pacientes valorados con Score de Alvarado tienen alta concordancia con el resultado de examen histopatológico de apendicitis, por lo que es de utilidad para el diagnostico temprano y oportuno de apendicitis
ABSTRACT: Background: The implementation of the Score decreases error appendicitis diagnosis. Material and methods: We performed a cross-sectional study, observational, descriptive and prospective, to determine the usefulness of the application of Alvarado score in patients with appendicitis, admitted to the emergency department of pediatrics HGZ /UMF No. 8 “Dr Gilberto Flores Izquierdo” in the period May 2009 to November 2009 The variables were analyzed using measures of central tendency and dispersion for quantitative variables and percentages for qualitative variables, positive predictive value, negative predictive value and ROC curve
Results: We conclude that the patients assessed with Alvarado Score have high concordance with the results of histopathological examination of appendicitis making it useful for early diagnosis and timely appendicitis.
