ABSTRACT: Kawasaki disease (KD) is an acute febrile vasculitis of unknown etiology that occurs predominantly in infants and young children. Although acute signs and symptoms of this disease are self-limiting, coronary artery damage is develop in G15% to 25% of untreated children and is the most important complication after KD. Coronary artery lesions (CALs) involve a continuous spectrum ranging from dilatation to multiple giant aneurysms which might lead to myocardial infarction, sudden cardiac death, or ischemic heart diseases. The efficacy of the use corticosteroids in addition to IVIG as an initial treatment in the acute phase of the disease has been demonstrated regarding symptoms and laboratory parameters so far there is not enough evidence in relation to CALs. In severe cases where a strong treatment is required it is necessary to find immunological or inflammatory markers for determining in which patients the addition of corticosteroids to IVIG as initial treatment during the acute phase of KD is recommended. The study design consisted of a prospective, randomized, double-blind trial. The results suggest that the level of the IgM antibody is a good criterium to determine the advisability of administering corticosteroids in addition to IVGG as an initial treatment during the acute phase of KD, with the aim of decreasing the risk of developing coronary artery lesions.
RESUMEN: La enfermedad de Kawasaki (EK) es considerada una enfermedad rara de etiología desconocida que afecta principalmente a niños menores de 5 años. Se trata de una vasculitis sistémica de vasos sanguíneos de mediano calibre con afección predominante de las arterias coronarias dando como consecuencia la formación de lesiones como dilatación, ectasia o aneurismas, lo cual favorece el desarrollo de una trombosis coronaria, infarto al miocardio y muerte súbita por el infarto o por la ruptura de los aneurismas. En diferentes ensayos clínicos se ha demostrado la eficacia del uso de corticoesteroides (CE) sobre algunos parámetros clínicos y de laboratorio cuando se administra concomitantemente con la gammaglobulina intravenosa (GGIV), durante la fase aguda de la enfermedad. Sin embargo no se ha logrado demostrar su eficacia sobre el desarrollo de lesiones en las arterias coronarias. En este trabajo se desarrolló un ensayo clínico aleatoriazado, doble-ciego donde se demostró que la administración de GGIV más CE, disminuyen hasta el 50% la probabilidad de desarrollar lesiones en arterias coronarias en pacientes con niveles elevados de IgM en suero, durante la fase aguda de la enfermedad.
