Evaluación del costo beneficio entre cirugías laparoscópicas versus cirugías abiertas ginecológicas en la clínica de especialidades Churubusco

Abstract

RESUMEN: La cirugía laparoscópica ha tenido gran auge en los últimos años. Disminuye el trauma quirúrgico, y el tiempo de recuperación. Sin embargo, en México no es utilizada universalmente en los padecimientos ginecológicos probablemente por el costo elevado que representa para el centro hospitalario o la paciente. Nuestra hipótesis es que la cirugía laparoscópica ginecológica no es un evento más caro que la cirugía abierta, si se toma el tiempo de recuperación y las incapacidades médicas. El objetivo de este trabajo es comparar el costo beneficio entre ambos tipos de cirugías, realizadas en el 2016 en la Clínica de Especialidades Churubusco y su repercusión financiera. Material y métodos: Estudio cuantitativo, descriptivo y transversal de 10 pacientes ginecológicas operadas bajo técnica abierta y 10 bajo técnica laparoscópica, con el objetivo de evaluar el costo de ambas técnicas. Se analizan datos clínicos, tiempo de recuperación, e incapacidades médicas. Resultados: La vía laparoscópica presento menor tiempo de recuperación, así como disminución en tiempo de incapacidades brindadas por los médicos. El costo del material laparoscópico fue más alto, aunque el costo total del procedimiento por paciente fue menor vs. técnica abierta incrementando las utilidades del ISSSTE al comprar el equipo mediante un financiamiento bancario. Conclusiones: La técnica laparoscópica en ginecología aporta beneficios para los pacientes y presenta, además, un costo menor. Produciendo utilidades monetarias en un menor tiempo comparado con la técnica abierta. ABSTRACT: In Mexico, laparoscopic surgery is not universally used for gynecological diseases probably because of the high costs to the hospital or the patient. Our hypothesis is that laparoscopic surgery is not as expensive as traditional surgery if we took in consideration the recovery of the patient and the time that doctors give to rest at home after surgery. Aim: To compare the costs of open and laparoscopic gynecologic surgery made in 2016 in “Clínica de Especialidades Churubusco ISSSTE” and its financial repercussion. Material and methods: We performed a quantitative, descriptive and transversal study of 10 patients operated by open technique and 10 laparoscopic with the objective of evaluating the cost of both techniques. Clinical data, recovery time, and medical disabilities are analyzed Results: The laparoscopic approach showed a shorter recovery time, as well as a decrease in the time of incapacities provided by the doctors. The cost of laparoscopic material was higher, although the total cost of the procedure per patient was lower. Open technique increasing the profits of the ISSSTE when buying the equipment through a bank financing. Conclusions: The laparoscopic technique in gynecology provides benefits for patients and also presents a lower cost. Producing monetary profits in a shorter time compared to the open technique.
