Prevalencia de hepatitis b en médicos acupunturistas en el área metropolitana

Abstract

RESUMEN: Se ha asociado la transmisión de Hepatitis B con acupuntura desde hace años sin que se cuente con el respaldo científico de esta afirmación. La Hepatitis B es un grave problema de salud por las consecuencias crónicas que esta acarrea. El objetivo de esta tesina es mostrar la situación real entre la Hepatitis B y la acupuntura, para que el personal de los bancos de sangre tengan como mediar su criterio. Por lo antes mencionado, se realizó un estudio descriptivo, longitudinal, prospectivo, en el cual participaron 25 médicos acupunturistas de la zona metropolitana de la Ciudad de México que han practicado la acupuntura por lo menos por seis meses, que no se encuentran vacunados contra la hepatitis B, que no han padecido hepatitis con anterioridad, ni tuvieron algún otro factor de riesgo. Se recolectaron muestras entre el 12 de octubre de 2004 y el 27 de julio de 2005 y se les aplicó una prueba de hemoaglutinación pasiva inversa para la detección de antígeno HBs. El analisis se realizó mediante el uso de la prueba Z para comparación de poblaciones, usando como primer parámetro la estadística de la prevalencia entre los médicos quirúrgico y como segundo parámetro la prevalencia de la hepatitis B en Asia. El poder deseado de la prueba es 80% y un alfa del 5%, utilizamos un factor de corrección de Yates, para ajustar la Chi cuadrada, lo que hace la prueba más conservadora y reduce la conclusión falsa positiva. Los resultados fueron de que no se encontraron médicos infectados, por lo que podemos decir que la prevalencia de la hepatitis B entre los médicos acupunturistas es menor de lo que se supone y que la relación que se considera entre la acupuntura y la hepatitis B es menos frecuente de lo que se cree.

ABSTRACT: The transmission of Hepatitis B has been associates with acupuncture years ago with out having scientifical backing for this affirmation. Hepatitis B is a serious because of the consequences it carries. The objective of this study is to find the real situation between Hepatitis B and acupuncture, so that the personnel have a way to rule their judgment. Because of what was stated previously, a descriptive, longitudinal, prospective study was made, in which 25 acupuncturist physicians of the metropolitan area of Mexico City that have practiced acupuncture for at least for six months, that have not been vaccinated against hepatitis B, that have not had hepatitis previously, nor have any of the risk factors took part. The samples were taken between October 12th, 2004 and July 27th, 2005 and they were applied a passive inverse hemoagglutination test for the detection of HBs. The analysis was made with the usage of the Z test to compare populations, using as first parameter for the statistics the prevalence between operating physicians and as second parameter the prevalence of hepatitis B in Asia. The desired power of the test is 80% and an alpha of 5%, we used Yates correction factor to adjust square Chi, what makes the test more conservative and reduces the risk of a false positive conclusion. The result were that we found no infected physician, so we can say that the prevalence of hepatitis B between acupuncturist physicians is lower than it is supposed and the relation between hepatitis B and acupuncture that has been considered is less frequent than it is thought.
