ABSTRACT: Obesity is a multifactorial problem that involves environmental and genetic factors. Particularly, G-2548A polymorphism in the promoter of leptin gene, an hormone involved in appetite regulation and energy expenditure, has been related with obesity and leptin levels in patients serum in different populations, but there is no data about Mexican population. In this work, we evaluated whether there is a relation between G2548A polymorphism in the promoter of leptin gene, obesity and leptin levels, in a sample of mexican patients (160), which were recruited in the acupunture clinic of the National School of Medicine and Homeopathy. We collected blood samples for DNA extraction and the measure of leptin levels in serum. The subjects were genotyped by PCR and enzymatic restriction using CfoI restriction enzyme. The AG genotype was the most frequent in women, while the GG genoptype was the most frequent in obese men, therefore G alele was the most frequent; nevertheless the statistical analyses showed that there is no association between G-2548A polymorphism and phenotype in the general population. However, we found a possible association (p = 0.059) between the G allele and the obesity in women. Then, we determinated the serum leptin levels through ELISA assay in our patients. In general, we observed that leptin levels were higher in obese women (47.1 ± 18.9 ng/mL); particularly the levels were higher in obese women with GG genotype. A comparative analysis of the leptin levels average in obese and lean women with the different genotypes at -2548 position in the promoter of leptin gene, using ANOVA statistical analysis, showed that GG and AG genotypes were related to higher leptin levels (p = 0.051). Finally, by a binary logistic regression analysis, we determinated that the risk of being obese was 3.7 times higher when the GG genotype was present (p < 0.001).
RESUMEN: La obesidad es un problema multifactorial que implica factores ambientales y genéticos. En particular, el polimorfismo G-2548A en el promotor del gen de la leptina, una adipocina involucrada en la regulación del hambre y gasto energético, ha sido relacionado con la obesidad y los niveles de leptina en suero en distintas poblaciones, pero no existen datos en la población mexicana. En este trabajo evaluamos si existe una relación entre el polimorfismo G-2548A en el promotor del gen de la leptina, la obesidad y los niveles de leptina en sangre, en una muestra de pacientes mexicanos (160), quienes fueron captados en la clínica de Acupuntura de la Escuela Nacional de Medicina y Homeopatía. Se colectaron muestras de sangre para la extracción de ADN y la medición de niveles de leptina en suero. Los sujetos fueron genotipificados por PCR y restricción enzimática usando la enzima CfoI. El genotipo AG predominó en mujeres, independientemente del fenotipo, mientras que el genotipo GG predominó en hombres obesos, por lo que tanto en mujeres como en hombres, el alelo G fue el de mayor predominancia, aun si los análisis estadísticos mostraron que no hay asociación entre alguna de las variantes genotípicas del polimorfismo G-2548A y el fenotipo en la población general. Sin embargo, encontramos una posible asociación (p = 0.059) entre el alelo G y la obesidad en la población de mujeres. Por lo que determinamos los niveles de leptina en suero mediante ELISA en estos pacientes. De manera general, observamos que los niveles de leptina fueron mayores en mujeres obesas (47.1 ± 18.9 ng/mL); particularmente, los niveles fueron mayores en mujeres obesas con el genotipo GG. Un análisis comparativo de las medias de los niveles de leptina en las mujeres obesas y delgadas con los diferentes genotipos en la posición -2548 en el promotor del gen de la leptina, mediante la prueba estadística ANOVA, mostro que los genotipos GG y AG se asocian con altos niveles de leptina (p = 0.051). Finalmente, por análisis de regresión logística binario, determinamos que el riesgo de ser obeso fue 3.7 veces mayor al presentar el genotipo GG, con respecto a no presentarlo (p < 0.001).
