RESUMEN: 1.- TÍTULO.
“Prevalencia y grado de depresión en pacientes con síndrome isquémico coronario agudo, (SICA) en un servicio de urgencias.”
2.- ANTECEDENTES: En el año 2006 en el Hospital HGR N. 12 del IMSS en la ciudad de Yucatán, Quintana Roo, México, se realizó un estudio con una muestra pequeña de 56 pacientes, en donde se incluyeron aquellos con cardiopatía isquémica en condiciones de responder el Inventario de Depresión de Beck, encontrándose una relación con la enfermedad coronaria en pacientes que han sufrido un infarto agudo al miocardio; una prevalencia mayor a la descrita del 14-47%.(13,28)
Así como en la ciudad de Torreón Coahuila, México en ese mismo año, en el hospital U.M.A.E. H.E. N° 71, del IMSS se estudiaron 139 pacientes consecutivos con síndrome isquémico coronario agudo (SICA). La prevalencia reportada durante los primeros días posteriores al SICA, de ésta muestra (8.6-31.2%) difiere a lo reportado en la literatura, 15-40%. Sin embargo, el trabajo fue realizado en los meses cuando se presenta con mayor frecuencia la depresión, (Octubre, Noviembre y Diciembre) siendo en un momento determinado un sesgo de manera significativa los resultados obtenidos. (20).
3. MARCO TEÓRICO: Se define depresión como un sentimiento persistente de inutilidad, pérdida de interés de un paciente por lo que le rodea y de falta de esperanza en el futuro. (15). En México, la Encuesta Nacional de Epidemiología Psiquiátrica encontró una prevalencia de depresión del 9.1% (17). En 1997 en México más de 4 millones de habitantes sufrían depresión. Para tener una apreciación de la magnitud de este problema a nivel mundial es útil considerar las tasas de depresión en instituciones psiquiátricas, hospitales generales y población general. En enfermedades crónicas degenerativas, particularmente en la cardiopatía isquémica, se ha observado una frecuencia elevada de manifestaciones depresivas que va del 17-27%, lo cual incrementa el riesgo de eventos vasculares y el fallecimiento.
4. JUSTIFICACIÓN: La valoración para depresión y ansiedad no se realiza de manera rutinaria en el servicio de Urgencias de la UMAE CMN La Raza, “Dr. Gaudencio González Garza”, en los pacientes hospitalizados por cardiopatía isquémica, como lo recomienda La Asociación Americana del Corazón (AHA). Por este motivo se realiza este estudio para determinar la prevalencia de depresión en sujetos hospitalizados en el servicio de urgencias con el diagnóstico de síndrome coronario agudo (angina inestable, infarto de miocardio con o sin elevación del ST) con la aplicación del Inventario de Depresión de Beck (IDB).
La cardiopatía isquémica es la primera causa de morbilidad y mortalidad en los países desarrollados y por su parte la patología afectiva constituye el principal motivo de consulta psiquiátrica en atención primaria con carácter claramente emergente.
5. OBJETIVO: Determinar la prevalencia y grado de depresión en pacientes con síndrome coronario agudo (SICA), en la sala de urgencias del HGGG CMN “La Raza” en el período de agosto-diciembre del 2009.
6. CLASIFICACIÓN DEL ESTUDIO: Es un estudio Transversal, Prospectivo, Observacional, Descriptivo.
7. VARIABLES: A. Variables Independientes: Inventario de Depresión de Beck I, Síndrome isquémico Coronario Agudo. B. Variables Dependientes: Depresión Leve; Depresión Moderada; Depresión Severa, Edad, Sexo.
8. PLAN DE ESTUDIO: La encuesta será aplicada por el investigador a todos los pacientes que ingresen con el diagnóstico de síndrome isquémico coronario agudo, establecido por los médicos adscritos al servicio de Urgencias Adultos de la Unidad Médica de Alta Especialidad “Dr. Gaudencio González Garza” Centro Médico Nacional “La Raza”, en el período de Agosto a Diciembre del 2009. Los criterios de inclusión fueron: Mayores de 16 años de edad, sexo tanto masculino como femenino; pacientes con diagnóstico de síndrome isquémico coronario agudo (angina inestable, infarto agudo al miocardio con desnivel del ST y sin desnivel positivo del ST), que se encuentren en el área de observación del servicio urgencias adultos; sin presencia de deterioro neurológico, se encuentren en condiciones físicas para contestar el Inventario de Depresión de Beck, mismo que se aplicará cuando el paciente se encuentre asintomático, sin datos anginosos y/o estable, previamente con consentimiento informado.Los criterios de exclusión: pacientes no cooperadores para la realización del Inventario para Depresión de Beck, pacientes con deterioro neurológico y/o que se encuentren con apoyo ventilatorio mecánico, pacientes con alteraciones del estado mental.
Se aplicará el Inventario de Depresión de Beck, así como la recolección de datos (anexo 2), para la realización del estudio, los resultado pudiendo ser: sin depresión, depresión Leve, depresión Moderada y depresión Severa. Aquellos pacientes que resulten con algún grado de depresión, serán enviados al servicio de Higiene Mental a su egreso de hospitalización, para complementación diagnóstica y valoración de probable inicio de tratamiento. La Muestra no probabilística para estudios con Variable Principal es de tipo Cualitativo. n = (Z)2(pq)/ (d)2, tomará un IC del 90%, con un error estándar del 0.1% (d) n=56.7 pacientes.
9. ASPECTOS ÉTICOS: Se respetarán los postulados de la Declaración de Helsinki para la investigación en humanos, solicitando autorización escrita en carta consentimiento. (Anexo 1).
10. RECURSOS E INFRAESTRUCTURA: El material humano y los consumibles a utilizar serán aportados por el investigador en su totalidad.
11. TIEMPO A DESARROLLARSE: Se pretende la realización de las encuestas durante los meses de agosto-diciembre de 2009.
12. CONCLUSIONES. Concluyendo que en los pacientes con síndrome isquémico coronario agudo, que se analizaron en el presente estudio, se encuentra un cierto grado de depresión mayor al reportado en la literatura consultada, por lo que es importante realizar medidas de prevención y detección oportuna. Así como la valoración por el servicio de Higiene mental en aquellos pacientes con diagnóstico establecido de Síndrome Isquémico Coronario Agudo, como se encuentra normado en la AHA y no esperar situaciones de riesgo.

For this reason we have this study to determine the prevalence of depression in subjects hospitalized in the emergency department with a diagnosis of acute coronary syndrome (unstable angina or myocardial infarction without ST elevation) to the implementation of depression inventory beck (DIB). Ischemic heart disease is the leading cause of morbidity and mortality in developed countries and in turn emotional pathology is the main reason for psychiatric consultation in primary care as an emerging clearly
Objective: to determine the prevalence and degree of depression in patients with acute coronary syndrome in the emergency room HGGG CMN "La Raza" in the period from August to December 2009. Classification of study: is a prospective, observational, descriptive study.
Variables: a. Independent variables: beck depression inventory, acute coronary syndrome. B. Dependent variables: depression mild, moderate depression, severe depression, age, sex
Study plan: the survey will be administered by the investigator in all patients admitted with the diagnosis of acute coronary ischemic syndrome, established by doctors attached to the ER adult medical unit of high specialty “Dr. Gaudencio Gonzalez Garza”, CMN “La Raza" in the period from August to December 2009. Inclusion criteria were: over 16 years of age, both male and female gender, patients with acute ischemic coronary syndrome (unstable angina, acute myocardial infarction with ST slope and ST without positive slope), which are in the observation area of the adult emergency service, without the presence of neurological impairment, they are physically able to answer the beck depression inventory, same to be applied when the patient is asymptomatic, no data angina and / or stable, previously with informed consent. Exclusion criteria: patients failing to cooperate in the completion of the beck depression inventory, patients with neurological impairment and / or encountering mechanical ventilatory support, patients with mental status changes. Apply the beck depression inventory and data collection (annex 2) to conduct the study, the result may be: no depression, mild depression, moderate depression and severe depression. Patients, who prove with some degree of depression, will be sent to the mental health service to their hospital discharge for diagnostic complementation and assessment of likely onset of treatment. The non-probability sample for studies with variable principal is qualitative. N = (z) 2 (pq) / (d) 2, will take a 90% ci, with a standard error of 0.1% (d) n = 56.7 patients.

Conclusions. Concluding that the conduct of this study, patients with acute coronary ischemic syndrome is a degree of major depression to that reported in the literature, so it is important to prevention and early detection. Just as the assessment by the department of mental hygiene in patients with established diagnosis of acute coronary syndrome, as it is ruled by the AHA and not expect risk.
Time to develop: the aim is to carry out the surveys during the months of August to December 2009.
